- Choosing to pay by check will allow you to mail your check along with a copy of this form (which you will receive via email) after successfully completing this form. You will be processed and have access to the Members Only section of this web site only after your check has been received and cleared.

Waiver
Annual membership fee $75 (non-refundable). Clinic fee $65 (non-refundable). All new members are required to attend an HGA certification clinic in order to participate in the tournament program.
Visa, MasterCard, American Express and Discover accepted. A 3% service charge applies to credit card orders.

Approval and consent by parent or guardian: As a parent or guardian of the applicant, I hereby certify the facts as stated in this entry form and attest that I am familiar with his/her plans to participate and that he/she does so with my approval. I further certify that, in the event that emergency medical care needs to be administered to the above named applicant and the below mentioned person cannot be contacted, the required parental consent may be given by an authorized member of the Houston Golf Association.
